The Population Health Sycle – Developing Population Health Management Programs – Step 1

The Population Health Sycle(TM) is a framework to developing Population Health Management programs that consist of seven steps, all starting with the letter S. These steps are Select, Survey, Segment, Sell, Solve, Score, and Start/Stop/Same. Each of these steps is essential to the successful implementation of population health management. In this blog post, we will take a closer look at the first step of the Population Health Sycle(TM), Select.

This step involves choosing the population that you want to manage. It is important to choose a specific population that has a common health issue or risk factor. For example, you might choose to focus on a population that is at risk for developing type 2 diabetes, or a population that has a high incidence of heart disease.

When selecting a population, there are several factors that should be considered. These include the size of the population, the demographics of the population, the health status of the population, and the availability of data. It is also important to consider the resources that will be needed to manage the population effectively.

A few recommendations:

Start small: It is better to start with a small population that you can manage well than to try to tackle a large population all at once.

Focus on the right population: The population you select should be at risk for poor health outcomes and should be accessible to your healthcare organization.

Get buy-in from stakeholders: It is important to get buy-in from all stakeholders in order to be successful.

The Select step of the PHC framework is a critical first step in managing the health of a population. By carefully selecting a population based on shared characteristics and risk factors, and gathering data to identify subgroups within the population, healthcare providers can develop targeted interventions that are designed to improve the health outcomes of each subgroup. Through ongoing evaluation and adjustment, the PHC framework can lead to improved health outcomes for the population as a whole.
