Audit Your Population Health Program: Is Your "Sycle" Out of Sync?
The transition to value-based care is a continuous loop, not a one-time project. With the Medicare ACCESS Model launch approaching on July 5th, it is the perfect time for medical groups and clinical leaders to audit their population health infrastructure.
If your outcomes are falling short, the friction often resides in a specific breakdown within the Population Health Sycle ™. To drive true clinical and financial transformation, every stage of your program must be tuned for performance.
The 9 Pillars of the Population Health Sycle
When I consult with organizations, we perform a "Sycle Audit" to identify exactly where the gears are grinding. Every step must be intentional and deterministic:
Select: Defining the target population and the specific health outcomes we are prioritizing.
Survey: Gathering the raw clinical, social determinants and other data needed to inform our population health strategy.
Segment: Organizing that data into actionable cohorts based on risk profiles and clinical needs.
Sell: Securing "buy-in"—ensuring patients are engaged in their care plan and clinicians are aligned with the proposed workflow change.
Solve: Applying evidence-based, interventions to generate change at an individual level to move the population as a whole.
Score: Measuring performance against predefined quality, financial and utilization metrics to gauge effectiveness.
And then we Sycle again based upon the data and:
Start: Initiating new interventions across the identified cohorts.
Stop: Discontinuing ineffective, redundant, or low-value interventions that drain resources without improving outcomes.
Same: Maintaining and scaling the interventions that have proven successful, ensuring consistency in the program.
Is Your Program Ready for July 5th?
Suboptimal performance in even one of these stages—especially a failure in the Sell phase—can lead to missed incentives and poor patient outcomes. The goal of Accountable Health is to help you "tune" this Sycle, ensuring your data infrastructure and clinical workflows are aligned with upcoming outcome-based payment models.
The Opportunity:
I am currently offering high-impact Sycle Audits designed to identify and fix these suboptimal areas before the mid-year regulatory shifts. We don't just find the gaps; we bridge them with the deterministic, transparent solutions that clinicians and patients actually trust.
